Accessing your health record

Your health record includes information like any conditions or allergies you have and any medicine you’re taking.

You can access your health record online and you can nominate someone you trust to access it too (this is called proxy access). We advise all patients to carefully consider granting access to another person as your record may contain sensitive, personal information.

You can use your NHS account (through the NHS website or NHS App) or Patient Access account to access your records. Offering access to patients records online became mandatory for all records from 1st November 2023. If you have an account but cannot see your records from this date and wish to, please contact us to investigate further.

For more information on how to register for online services, please see the managing your
healthcare online page.
